• If you are a passenger only, please check the Passenger Only box and list your rider’s name & phone.
• Membership Dues for passengers are optional. If you want to be Patched In, dues are required.
• Form submission does not guaranteed membership.  Forms will be reviewed by Club Officers before final determination.
